Our student’s final year with us can be an exciting yet worrying time for our families. I work very closely with our final year (Transition) students to ensure that the transition process is as smooth as possible. I also work alongside outside agencies to source the best placement for each individual student.
Throughout the year, events such as the Transition Morning and the Student and Parent Information Evening will be held so parents/carers can see for themselves the different types of provision they could potentially use and also if they need any medical advice or assistance with any other type of support. I also help to secure students a social worker should they need it for their transition and work alongside the social workers to allocate the most suitable placement for the student’
